The Collaborative provides individualized services to clients of any age

The Collaborative provides individualized support and guidance
to help others make necessary behavioral changes
to improve physical and mental health.

The Collaborative is a team of skillful and talented people who work together in an egalitarian fashion. We share ideas, information and problems to determine the best possible actions to help people move toward happier, healthier lives. Our group is easy on judging and sharp on doing the right thing, for the right reasons, at the right price.

We believe that current service providers in our area are doing great work, but we also believe that there are some gaps that can be filled. We want to be a solution to some of the missing services.

Our Mission
Assist and serve as requested and as needed by the diverse community of Albuquerque, especially those who want to change their life styles, or who might be ill, disabled or elderly. As we achieve our mission, our whole community becomes happier, safer, more peaceful, and more at ease.

Services We Provide
Our services are designed around our clients, and are provided wherever they are needed.

  • Therapeutic Programs providing cognitive, physical, emotional and spiritual activities to stimulate and energize clients.
    • Escort to community events, e.g., church, theater, concerts, political events, classes
    • Reading to clients or writing letters for clients
    • Involvement with Hospice patients, even to performing the Vigil
       
  • Personal Health Coaching - specific to a particular client to maintain, attain or retain good health.
    • Planning, supporting and encouraging client’s choices of activities to enhance happiness and health
    • Private coaching for behavior change to enhance quality of life for clients
    • Caring for pets for or with clients
    • Walking and exercising with clients
    • Meal planning, shopping and diet ideas
    • Visits or telephone check-in calls

Our Team

Lynne LaFountaine, Managing MemberLynne LaFountaine, Managing Member
Lynne has served elders and their families for more than a decade in New Mexico and for a decade before that, in Oregon. Her formal education is in psychology and special interests are in death and dying, mindfullness and yoga.
 

Judith A. Corey, Program SpecialistJudith A. Corey, Program Specialist
Judy specializes in creating meaningful life styles for the frail and elderly. She has a certificate in Aging Studies and a Master’s Degree in Professional Studies. Judy has spent the last twenty years loving every day she has spent working with the elderly in therapeutic group programs, geriatric care management as well as being a Nursing Home Administrator.
 

McKenna Minor, Wellness SpecialistMcKenna Minor, Wellness Specialist
McKenna has devoted her life to the healing arts. A Reiki master with more than thirty years experience, she is also an equine massage therapist and a professional photographer. McKenna is committed to supporting and encouraging emotional, physical, and spiritual wellness in others through her services.
